That by completing this Maintenance Request Form you grant permission for your apartment and room to be accessed by a member of the facilities team or, if necessary, a specialist external contractor within 3 working days.

It is also your responsibility to advise the other tenants within the apartment that you have authorised access. No further notice of intent to enter will be sent.
